The state of California requires a doctor’s referral to book with a dietitian, and I’m here to make the process as quick and easy as possible.

DO I NEED A REFERRAL IF I'M IN ANOTHER STATE?

I’m licensed to provide nutrition care for residents of Arizona, California, Colorado, Illinois, Michigan, New Jersey, Texas, and Virginia. Of those states, California is the only one that requires a physician referral (so if you're anywhere else, you're good! Just submit an inquiry here!)

Do I need a referral in California even if I don’t use insurance?

Yes, you do - it’s required to establish a relationship with a dietitian, but thankfully we only have to do it one time!

I’m having trouble figuring out this referral form or how to get my doctor to sign it.
